So we decided to run a rather unorthodox experiment to answer the question of whether the problem was all on baby's side or not. Mom had a very good friend with a 3 mo old baby and a great milk supply. We discussed the relative risks and they both were willing to go forward, so we had the moms swap babies to see how well my mom's breasts drained for a baby with no issues and to see how well our baby was able to get milk out of a normally draining breast. The end result a colleague labeled "the perfect storm." little of both. I would really, really like the take of those of you who are knowledgeable on suck and feeding issues before we make arrangements for treating baby's frenulum. http://www.youtube.com/watch?v=1OBPXyo1Yfo

Another colleague's report-

Just this week I saw a mom with a 4 month old baby girl. Everything was going fine for 2 months. Sometime in the 3rd month the baby started not wanting to nurse. Mom busy with older 2 year old- Not really noticing, thinking it was just a phase.Prolonged period between feeds. Fussy at breast. oral aversion. Nipple aversion. Will not take a bottle or anything else in mouth. Very fussy. She presents with a high anterior bubble palate. Gags when my finger goes in slightly. Not far at all!! And when sweeping under her tongue a "speed bump". will only brfd in her sleep, never awake. refuses to have anything in her mouth during wake times. Mom's milk is in the toilet. Mom is very upset and overwhelmed. Does not understand why this was not seen and taken care of at the hospital. Wants my help but not really.... so upset she really can't move in the direction of fixing this

This is not the first time I have seen a posterior TT in the 3rd or 4th month fall apart when everything was going well at the beginning. This is why I think maybe things were not necessarily going so well in the beginning. Baby working way too hard.. patient baby just hanging in there.. mom with high pain tolerance.. busy with other children. Remembering her friends and family saying "Oh it will hurt for at least 6 weeks". So when asked how things are going "well just fine". Maybe it was just fine for mom but maybe it is not just fine for baby. As I said before if it is a patient laid back kind of a baby then things may present themselves as just fine. Until the baby decides everything is NOT just fine
